4. Your mailing address including: Street number and name, Apartment # (if applicable), City, State and Zip Code
5. Your E-Mail address
6. The items you wish to purchase:
   1. Quantity
   2. Style (Please note product name(s) and/or style #)
   3. Size
   4. Color

Please do not leave out any information that is requested, this may delay the ordering process.
We will respond to you as quickly as possible with the order confirmation.
Following the confirmation email you will be sent an email from Paypal co/ MAD INDUSTRY.
Follow the link in the email. This invoice will be for the goods purchased minus your Groupon ($40.00 value). Click “continue”
On the next page you do not need a Paypal account, simply click on “continue” under the heading “Why Use Paypal” and you will be asked to enter in your information.
